As part of its national Crusade Against Cavities, FireFly toothbrush is launching a nationwide contest to honor pediatric dentists for their charitable work with kids.

Nominate your dentist by fill out the form below with a 100-word statement on why your dentist deserves the honor. The contest begins January 25, 2011 and runs through May 1, 2011. The winner will receive 10,000 toothbrushes to be donated to a dental charity of his/her choice and airfare and hotel accommodations to the American Academy of Pediatric Dentistry 64^th Annual Session in New York City in May 2011.
